  • One on one visits with a Care Worker in the comfort of your home.
  • Finding new activities that match your interests and engage with others.
  • Transport to visit a café for a coffee or to go shopping.
  • Attending local community events like football games, theatre, or other spiritual activities with like minded people.
  • Support to attend appointments eg. your doctor or hairdresser.
  • Helping you spend time with your friends at home.

Our team can help you take part in cultural activities and organise any translating or interpreting services you need. We can help you use technology to access services, manage everyday needs like paying bills, and stay connected. 

We can also link you with a vast network of local community groups. With a diverse range of interests from sport, cuisine, art and music, you can find new friends with similar interests. 

In Western Australia (WA) we have dedicated Social Centres designed to bring people together and to help you connect with others. Our centres provide a range of activities such as music, art and craft sessions, help with meal preparation, and social outings in the community.

We can also help you to stay connected by linking you with a vast network of local community groups. With a diverse range of interests from sport, cuisine, craft and musical events, you can be sure of finding new friends with similar interests.




Our services can be funded through the Commonwealth Home Support Program, the Support at Home program, or privately funded, whatever suits your situation.

Health professionals can refer patients for government funded aged care services by using the My Aged Care online referral tool, or by calling 1800 200 422. You can also make an e-referral from your practice management system.

For private services, patients can contact us directly. However, you can contact us on a patient’s behalf by calling 1300 300 122.
