If you need Government funding to help pay for your aged care services, you can choose a provider before, during or after you have been assessed for care. When choosing a provider, it is important to understand how much funding is available to you and whether the provider can deliver the services you need.

Changing your provider

By following these steps, you can change providers without the need for a new assessment, and without delaying your services.

1. Tell your current provider: Contact your current provider and agree on an end date for your services. Ask them to stop your services, not your funding.

2. Contact Silverchain: Let us know you'd like to switch to Silverchain. We will discuss your needs and explain our Service Agreement. We can also agree on a date for when your services can begin.

3. Create your Care Plan: Your Care Partner will work with you to develop a personalised Care Plan that supports your goals, preferences and care needs.

4. Start receiving our care: We will work with your current provider to ensure a smooth transition. Your services with Silverchain will start on the agreed date, with no break in your care.
