The Transition Care Program (TCP) provides funding for up to 12 weeks for services that will help you recover at home after a hospital stay.
If you’ve recently had a hospital stay, the Australian Government’s Transition Care Program provides funding if you need short term care after being discharged from hospital, to help support your recovery. You can access a team of qualified professionals, who can treat you at home. This may include a Nurse, Occupational Therapist, Social Worker, Dietitian, or Speech Pathologist. Working together, your care team can help you to live independently at home by providing:
- Support with Personal Care like showering and grooming.
- Help to reduce the risk of falling at home and in the community.
- Support to improve your confidence and reducing your anxiety when you return home.
- Access to medical appointments.
- Help to continue attending activities in the community that you enjoy.
- Arranging mobility equipment or services that you may need.
We provide TCP services in the Great Southern region of Western Australia (WA), South Australia, and in the Sunshine Coast Hospital and Health Service catchment area of Queensland (QLD).
Eligibility
To access government funding, you will need to be aged 65 and over, or aged 50 and over for Aboriginal and Torres Strait Islander people. You must also be a current hospital patient ready for discharge and have a need for short term support.
Services can be provided for up to 12 weeks in your home or in the community.
To access TCP funding and support, you will need to be assessed while you’re in hospital. You can ask your hospital staff to organise this, or you can schedule an aged care assessment through My Aged Care (MAC).

For private services, patients can contact us directly. However, you can contact us on a patient’s behalf by calling 1300 300 122.
