Refer to Primary Care at Home

Primary Care at Home is a free service designed to reconnect clients with the primary health services they need.

Our nurse practitioner will visit the client in their place of residence and will:

  • complete a comprehensive health assessment, work with the client on an individualised care plan and provide referrals to relevant services
  • connect the client with a regular GP and support the development of an ongoing relationship.

Is my client eligible?

The service is free to clients who:

  • live in metropolitan Perth
  • have health concerns such as troubled breathing, a heart condition or diabetes, and are not currently not seeing a GP or health professional regularly
  • are facing community disadvantage
  • currently receives support from Silver Chain or a partnered community organisation.

How do I refer a client?

Community Support Workers from partnered community organisations can refer eligible clients to Primary Care at Home. Download the referral form, or call us and ask for the Primary Care at Home Program Manager.
