Getting out and about, going to appointments, and doing the things you love keeps you happy and connected. It can get a little trickier to maintain that independence over time, which is why our care team is here to assist.
We offer door-to-door safe and reliable transport options to ensure that you can get to the places you want to go to, such as:
- joining in with social activities and community events
- doing your banking and shopping
- getting to and from appointments
- visiting family and friends.
Our transport may range from travelling in cars or minibuses, or assistance using public transport. We will make sure you travel to your destination/s and arrive home safely.
Who can access our services?
Our transport services are suitable for people who may need a little support with everyday tasks due to age, impaired mobility and illness.
Fees and pricing
If you or your loved one is aged 65 years and over, or are an Aboriginal Torres Strait Islander over 50 years, you may be eligible for government funding for aged care services through the Commonwealth Home Support Programme (CHSP) or through a Home Care Package. Services can also be self-funded for any person of any age.
As a Silverchain client, you will need to pay a contribution fee for our services based on the funding you receive. These fees help us to deliver more services to more people in Western Australia.
