Support to manage your medications, take them safely and as prescribed.

Our nurses can support you to remain confident and in control of your medication.  

We can provide reminders, develop plans to help you take your medication and can work with your GP or Pharmacist to review your medications and make any necessary adjustments.  

Visiting you at home means we can help you take your medication at the right time, correct way, in a routine, that gives the confidence that you're taking it safely. 

Our help can also include:

• Reminders and administration of various medications (including injections).
• Arranging your multi-dose packs, in line with your medication schedule and medication deliveries.  
• Monitoring your symptoms and side effects from taking your medication.
• Coordinating medication reviews with your GP and Pharmacist.
• Education on the medication you are taking.
