Hospital discharge support in Western Australia
Recovery support in your home
This government-subsidised short term program aims to get you back on your feet after a stay in hospital by working towards your goals, as well as helping you avoid a return to the hospital. This may include:
- Assisting you to live as independently as possible
- Improving your confidence and reducing your anxiety when you return home
- Reducing your risk of falling
- Reducing or removing your need for ongoing services or possible return to the hospital
- Preventing or delaying any further decline and promoting healthy ageing, keeping you as independent as you can for as long as you can.
How does it work?
You’ll meet with your care manager who will work with you to determine the support you need to manage at home and develop strategies to regain your independence. This might include working with our occupational therapists, physiotherapists, therapy assistants and care aides.
These services may be in place for up to 6 weeks as part of this program, and may include:
Short term strategies
Helping you transition home with useful strategies such as providing task simplification techniques for personal care and home tasks, falls prevention education, or assistance purchasing and using equipment and aids.
Long term strategies
Providing you with strategies to keep you healthy longer term such as exercises to regain strength and balance, increased physical activity, education to self-manage ongoing conditions, and accessing information, community resources and programs.
Ongoing needs
Once the program has finished, we’ll work with you to make sure you can access further services and support if you still need it. This may be through other government-subsidised programs, such as the Commonwealth Home Support Programme.
Who is it for?
- Recent illness or surgery
You need to still be in hospital and medically stable after a recent illness or surgery. - Short term support
You need short term support to regain your ability to complete normal daily tasks when you return home. - Selected regions of Western Australia
You live in the Perth metropolitan area, or in the South West or Greater Southern region.
The best thing to do is to talk to hospital staff while you’re still in the hospital to see if Hospital Discharge Support is suitable for you. If it is suitable and you consent, hospital staff will refer you to us while you’re still in the hospital. You are no longer eligible for this program once you’ve left the hospital.
