A Home Care Package (HCP) is a funding package from the Australian Government designed to subsidise the cost of in home aged care for older Australians.

A HCP is often the next step on from receiving basic home care services through the Commonwealth Home Support Programme (CHSP). This is because it provides funding for you to receive a higher level of supports and care.

Depending on your needs there are four levels of care that you can receive funding for. These range from a Level 1, which may include home and garden maintenance, to Level 4 for complex care like nursing and allied health care.

Silverchain provides the full range of HCP funded services across all 4 levels of care. These include:

    Support with daily tasks like showering, getting dressed, and taking your medications.

    Help with your grocery shopping, preparing meals, and making sure you eat well.

  • Social Support and Companionship
    Support to keep you socially active, connected, and independent.

  • Transport
    We can drive you or support you to travel to your appointments, the local shops, or other places of interest.

  • Allied Health Services
     Support to restore and maintain your mobility and improve your independence at home.

  • Nursing Services
    Tailored care to help manage and support your health care needs.

Managing your package

With a HCP, you can choose to have your package managed by your provider. At Silverchain we generally offer provider managed care to our clients. If you’ve chosen us to manage your care, you will have a dedicated Care Coordinator or Case Manager to help you do this.

Refer to us

Health professionals can refer patients for government funded aged care by contacting My Aged Care, or by calling 1800 200 422. You can also make an e-referral from your practice management system.

For private services, patients do not require a referral or assessment and can contact us directly. However, you can contact us on a patient’s behalf by calling 1300 300 122.
