Community Nursing

We offer skilled nursing care at home, supporting patients recovery and sustained health and wellbeing.

At Silverchain, our qualified nurses deliver personalised care in the home to help your patients recover from illness, injury, surgery, or hospital admission. We collaborate with families and carers to ensure continuity of care and improved outcomes.

Nursing Services funded by Commonwealth Home Support Program (CHSP)

Our Nursing Services funded by CHSP are for non-acute, planned, predictable, regular care. Clinical care available includes:

• Regular catheter care.
• Assessment and care for skin tears.
• Administering regular injections for insulin and other medications (except immunisations).

Complex Nursing

A community based complex nursing service designed to reduce demand on hospitals. Patients are supported through the provision of shared care, with our nursing team working together with their referring general practitioner (GP) or other medical specialists (eg urologist). Clinical care and treatments available include:

• Complex wound care.
• Lower leg and foot ulcers.
• Acute and chronic wounds.
• Complex medication management.
• Trial of void and complex SPC catheter management.
• PICC/PORT ongoing management care.

St Patrick's Centre (Fremantle, WA)

We also provide a nursing clinic at St Patrick’s Community Support Centre, supporting vulnerable people in the community who are homeless, or at risk of homelessness. Services include clinical assessment, care coordination, chronic disease management, wound care, and diabetes care.
