Aligned to a shared purpose, our partnerships with governments, health networks and private providers extend our impact and uplift our capability to deliver on our ambition to create a better home care system for all Australians.

In 2022-23 Silverchain partnered with four Primary Health Networks (PHN) across Australia to deliver care coordination services to people living with long term health conditions such as diabetes, asthma, arthritis, heart of lung disease, cancer, and back issues.

Operating in the regions of South Eastern Melbourne, South Eastern New South Wales, Brisbane North and the City of Wanneroo in Western Australia, our experienced care coordinators work with local GPs and stakeholders to help clients manage their health, social and wellbeing needs to help keep them out of hospital.

Personalised to suit the needs of each client, our teams helped our clients to make and keep appointments, fill prescriptions, find social and funding support, and educated them on managing their condition to achieve their health and lifestyle goals.

Silverchain signed a memorandum of understanding with the ARC Centre for Innovation and Sheba Beyond, one of the world’s first virtual hospitals – a collaboration that will accelerate digital health innovation in home care. The partnership will strive to drive growth and drastically improve the way care is delivered to homes across Australia. Many of our partnerships are long standing and reflect our mutual commitment to the communities we serve. Our partnership with Adelaide’s Hutt St Centre as RDNS Silverchain’s Homeless Nursing Clinic started more than 30 years ago. The service began in one small room, but in the 2022-23 financial year it supported more than 1,400 people experiencing or at risk of homelessness access primary health care.

Our Community Impact & Inclusion Strategy included a focus on our partnerships with and for the LGBTQIA+ community. As we progress toward becoming a Rainbow Tick accredited organisation, Silverchain joined Pride in Health and Wellbeing to provide support, resources, training and consulting.

This included supporting Silverchain to launch its Silver Pride Ally Network in June 2023. The Ally Network is a group of employees who undergo training to become allies and advocates for the LGBTIQA+ community, offering support, resources, and a safe space for employees to be themselves. By establishing this network, we will further foster an environment of respect and inclusion for all employees, regardless of their gender identity or sexual orientation.

This year we also joined the Australian Network on Disability. Through this collaboration and knowledge sharing with other members, our aim is to enhance accessibility and infrastructure, policies and practices, to ensure every individual, regardless of their abilities, receives best care, every time.
