At Silverchain, we offer a free Care Plan Support and Coordination Service for adults living in the City of Wanneroo who have mental health and/or drug and alcohol issues. You may also be living with long term health conditions such as asthma, back pain, cancer, heart, or lung disease.

How we help

Our service helps you get the most out of life and stay out of hospital as we work closely with you and your GP to provide support depending upon your health, social and wellbeing needs. Our service can include:

  • Finding mental health and/or alcohol and other substance use support services.
  • Help to find social support like financial, relationship or grief counselling services.
  • Booking appointments and arranging transport.
  • Finding other solutions when you face long wait times for the services you need.
  • Providing advice and information on what you need to manage your health well.
  • Explaining medical language and advice, as well as test results to you.

 This service is funded by WA Primary Health Alliance through the Australian Government Primary Health Networks Program.
