Transition care packages
Available to people living in the Great Southern Region of Western Australia, a transition care package can help you regain your independence after a stay in hospital.
Transition care packages are short-term packages of care designed to help you regain your functional abilities, both in and around the home, after a stay in hospital.
We have a wide range of services available to assist you during your recovery, so that you can get back to your everyday activities. This includes:
- a dedicated care manager who will co-ordinate all aspects of your package, including establishing community support and services and helping you explore your care options after the package ends
- low intensity therapy such as physiotherapy and occupational therapy
- nursing support
- personal care
- domestic help.
This is usually a short term, intensive program that is focused on helping you improve or regain your level of independence. It’s often called ‘reablement’ or ‘restorative care’.
How do I get this service?
Transition care packages are only offered to people living in the Great Southern country region of WA. You will need to be assessed by an Aged Care Assessment Team (ACAT) to receive these services.
How much does it cost?
You will be asked to contribute to the cost of your care through a daily fee.
