Short-Term Restorative Care in Victoria

We are challenging and advocating for an integrated care approach for all Australians in the health, community and aged care sectors. Whether at home or facilities based, we work to provide access to quality in-home care.

While we don't currently provide Short-Term Restorative Care Program services in your area, we are committed to ensuring that people have access to the right information regarding their Short-Term Restorative Care Program options where and when they need it.  

We have compiled a list of considerations that we believe are important for you when seeking care. We hope this helps you on your care journey and as we grow and expand, so will our ability to provide more choice for quality home-based care in your area.

Regain your independence 

Short Term Restorative Care services can include a range of services that are specifically designed to help you improve your strength and confidence that helps keep you living independently. Your provider should work with you at home or in the community for up to 8 weeks to get you back to where you want to be.

Getting started

Your provider’s allied health team should work with you to understand what’s important to you, and identify your needs and goals.

Develop a tailored plan 

A tailored plan should be developed to help you meet your goals, including strategies so you can get back to doing what you love to do.

One to one support 

Occupational therapists, physiotherapists and podiatrists can work with you on a one to one basis either at home or in the community, so it’s always all about you and achieving your goals.

Home support services 

Home support services can include personal care, shopping assistance and help at home, providing you with temporary support while you get back to normal.

Equipment and home modifications 

Sometimes making small changes to your home can help you with your daily tasks and activities. This may be installing rails or ramps, or equipment such as walkers or hearing and visual aids.

Short-Term Restorative Care in Western Australia

If you have recently experienced new or changing health conditions, or if you have recently been to hospital you may find new challenges in day to day living. That's where our caring expert team can provide you with the support you need to get things back on track.

The goal is to deliver you completely personalised services tailored to help you regain or improve your independence, so you can continue to live confidently and independently in your own home.

Our packages are:

  • designed to help you achieve your set goals
  • delivered by a team of people working collaboratively
  • provided in your home or in the community
  • short-term (up to eight weeks).

We provide Short Term Restorative Care to people living in the Great Southern and South West country region of WA, delivered from our Bunbury, Busselton and Albany service centres.

What can the program include?

Our Short Term Restorative Care program can include a range of services that are specifically designed to help you improve your strength and confidence that helps keep you living independently.

Allied health and therapy services

We offer allied health services such as occupational therapy, physiotherapy and podiatry at home and in the community, to help you start achieving your goals.

Home support services

Our home support services include personal care, shopping assistance and help at home, providing you with temporary support while you get back to normal.

Equipment and home modifications

Sometimes making small changes to your home can help you with your daily tasks and activities. This may be installing rails or ramps, or equipment such as walkers or hearing and visual aids.

Eligibility

Your GP or hospital-based health professional can refer you for an assessment, or you can organise an Aged Care Assessment Team (ACAT) assessment by contacting our team or My Aged Care.

Cost
These services will be fully funded with a completed and approved assessment.
