Our goal is to keep people out of hospital as much as possible, ensuring they receive respectful, professional, comprehensive care, where they are most comfortable.

Care Coordination Support Service

The Care Coordination Support Service is a free service supporting people living with chronic conditions to effectively implement care plans provided by their GP, while also supporting their social needs.

Referral criteria

Eligible:

  • Ages 18 and over, or 15 and over for Aboriginal and Torres Strait Islanders
  • lives in one of the following local government areas (LGAs) - Glen Eira, Port Phillip, Mornington Peninsula, Casey, Stonnington, Greater Dandenong, Kingston, Bayside, Cardinia, Frankston.
  • has a chronic condition or complex needs such as asthma, chronic pain, diabetes, cancer, heart or lung disease.
  • has limited access to multidisciplinary/health care.

Not eligible:

  • where the answer is No to any of the above criteria, or is a:
    - current recipient of HARP (Hospital Administration Risk Program)
    - DVA cardholder eligible for care coordination services through DVA
    - NDIS clients
    - Workcover client or under Transport Accident Commission
    - permanent resident of a residential aged care facility
    - recipient of palliative care services.

Cost

No cost to patient.

Program is funded by the South Eastern Melbourne Primary Health Network (SEMPHN)
