Care Management is an important service that ensures the quality and safety of your care.
If you are receiving government funding for your aged care services, through the Support at Home program, or funding your care privately, you will have a dedicated and experienced Care Partner.
Your Care Partner will make sure your care is safe, effective, and respectful of your preferences, culture, spiritual needs, and identity. They will work with you, your family and your health care providers to create your Care Plan.
Your Care Plan will include your goals, how you would like your care delivered, and the services you will receive. It will be reviewed regularly, to ensure that it matches your changing needs over time. Your Care Partner will also help you to access resources or other services like advocacy support, to make informed decisions about your care.
You will also have a Restorative Care Partner. They will work together with your Care Partner who manages your ongoing Support at Home services, and your doctors to ensure your care is consistent.
Provider v self-managed care
At Silverchain, we believe that it is important to get the most out of your aged care services. That is why we only offer Care Management, provided by our Care Partners who have extensive industry experience, who will take the stress out of having to plan and coordinate, the services and support you need.
Care Management
Care Management is a service every person with Support at Home funding receives, regardless of your provider. Services Australia deducts 10% of your quarterly Support at Home budget and adds it to your providers Care Management Account. This deduction will occur even if you choose to self manage your own care.
You may be able to apply for additional Care Management funds if you have specific or complex support needs. This can include being at risk or experiencing homelessness, an Aboriginal or Torres Strait Islander, or a veteran with mental health conditions.

Refer to us
Health professionals can refer patients for government funded aged care by contacting My Aged Care, or by calling 1800 200 422. You can also make an e-referral from your practice management system.
For private services, patients do not require a referral or assessment and can contact us directly. However, you can contact us on a patient’s behalf by calling 1300 300 122.
