Welcome to the Silver Chain Referral Information portal for Broome Health Campus. 

Silver Chain is a not-for-profit community health and care organisation that has been supporting people to live independently in the comfort of their own home for 110 years.

The partnership between GPs/ key stakeholders and HACC is crucial to delivering the best care and outcomes for patients. If you have a patient who's eligible for HACC Nursing, here's how to refer them.

Referring your patients is easy

Telephone: 1300 650 803 and speak directly to our Case Coordinating Nurse (CON) to discuss your referral and determine if your patient is eligible. 

Fax: Alternatively, download, complete and fax in one of our referral forms listed below to (08) 9444 7265. One of our CONs will call you back to discuss your referral. 

Patient Eligibility

A Person May be eligible for the WA HACC program support if they:

  • are older and frail and are having difficulty meeting everyday tasks including accessing their community
  • have a disability that impacts on their ability to undertake everyday tasks
  • have a low level of support in Nursing that does not exceed a level 2 package
  • have a clinical need within the scope of the service such as wound/catheter care and medication management such as diabetes
  • where patients are not eligible support may be negotiated to purchase the service where capacity exists at full cost recovery
  • where there is a short term clinical need and no other service available.
