How to get a Home Care Package
Find out how to access a Home Care Package to help you live a healthy and safe life at home for as long as you choose.
If you’re a senior who wants to continue living at home, but you need regular or co-ordinated support to do so, then a Home Care Package may be the perfect solution for you.
1. Find out if you’re eligible
Before you can receive a Home Care Package, you will need to be assessed by the Aged Care Assessment Team (ACAT).
Assessments are organised through My Aged Care, the national aged care navigation portal. They talk to you over the phone and will refer you for a free assessment.
To arrange an assessment, you can :
- give us a call and our friendly team can help get you started
- register with My Aged Care, by calling them on 1800 200 422 with your Medicare number ready.
2. Have an assessment
An independent assessor will come to your home to get an understanding of your needs.
With a Home Care Package:
- you will be allocated one of four package types – level 1, 2, 3 or 4 – depending on the complexity of your support needs
- you have control over your funds and can decide how to spend it
- once you have a package, it stays in place for the long term.
After your ACAT assessment, you will receive a letter to let you know if you have been approved to receive a Home Care Package. This letter will also confirm the level of package you are eligible to receive. You are then automatically placed in the government queue for a Home Care Package.
3. Set up your Home Care Package
When you’ve reached the top of the queue, you’ll receive another letter which confirms you have been assigned a package and will include your unique referral code.
From here, you have 56 days to select a provider.

One of our care team members will visit you at home and, together, we’ll work with you to design a care plan that’s tailored to your needs, budget and personal circumstances. From here your services will commence.
Getting to know you is important to us. It means that we understand your goals and preferences and, as your needs change, we can change with you.
There is no cost to have an assessment for a Home Care Package, but if you are eligible, you may be asked to contribute to the cost of your care, where your personal circumstances allow.
