Personal Enablement Program (PEP)
If you have recently spent time in hospital and need some extra help to get up and go again, you might be eligible for our Personal Enablement Program (PEP).
PEP is a short term program specially designed for usually independent people who are about to be discharged from hospital. PEP can assist you to regain - and even boost - your wellbeing and independence - physically, mentally and socially.
Our team of occupational therapists, physiotherapists, registered nurses, therapy assistants and care aides can work with you to help you regain your confidence and strength, so you can remain living at home safely and effectively. Care is tailored to the need of each individual and designed to empower you to get back on your feet and achieve your highest level of independence.
We will support you to work towards your own goals and help you reconnect to your community.
How to get this service
You need a referral to participate in our Personal Enablement Program. Your discharge coordinator or social worker can arrange a referral for you.

If you participate in this program you will be required to pay a contribution towards your care.

How to refer a patient to PEP
Hospital referrals can be made during business hours Monday - Friday by calling the Silver Chain Contact Centre on 9242 0347. PEP referrals will be screened by the Allied Health Liaison, who will discuss with the referrer the individual needs of the client.
