Hospital Discharge Support
From July 1st, the West Australian Government transitioned Home and Community Care (HACC) services to the Commonwealth Home Support Programme (CHSP), which is part of the national aged care system. This provides a consistent system for people to access aged care services, no matter where they live in Australia.
Changes to the funding mean we will no longer be accepting referrals to the Personal Enablement Program (PEP) from hospitals.
Our new reablement service, Hospital Discharge Support (HDS), was introduced from 1 July 2018, replacing PEP. Hospital Discharge Support is our new rapid response service which will provide necessary support in the community to prevent readmission to hospital. Hospital Discharge Support is a short-term program, with the duration that each client spends in the program assessed by our allied health team depending on their needs. We are committed to supporting a smooth transition for clients who require further support and will refer and transition clients to other community based funding models when appropriate.
Our Allied Health team will continue to deliver services with the same restorative focus to support clients to delay or avoid readmission or having to move into residential aged care.
To be eligible for Hospital Discharge Support you must:
- be in hospital at the time of the referral
- be medically stable
- reside in an area where Hospital Discharge Support is available.
