Hospital Discharge Support
Hospital Discharge Support is our new reablement service. It has replaced the Personal Enablement Program (PEP) as a short-term support program for when you return home from hospital.
Hospital Discharge Support is rapid response service which provides support to you in the community. It will help you recover from a recent illness or surgery.
Our allied health team will assess you in your home and design a short-term program around your own goals. The length of time you will spend in the program depends on your individual needs.
Our aim is to get you back on your feet and help you avoid a return to hospital.
Recent changes to personal enablement
Since 1 July, Home and Community Care (HACC) services have transitioned to the Commonwealth Home Support Programme (CHSP), which is part of the national aged care system. This provides a consistent system for people to access aged care services no matter where you live in Australia.
This change means we will no longer be accepting referrals to PEP from hospitals.
We are committed to supporting a smooth transition for clients who require further support and can help transition you to other community services when needed.
Our allied health team continue to work with the same restorative focus. Our aim is to support our clients to delay or avoid re-admission to hospital or having to move into residential aged care.
To be eligible for Hospital Discharge Support you must:
- be in hospital at the time of the referral
- be medically stable
- reside in an area where Hospital Discharge Support is available.
