Home Independence Program (HIP)
Our Home Independence Program (HIP) is a short term program designed to help improve your function and confidence to undertake daily tasks, especially after a fall.
Our team of HIP coordinators, physiotherapists, occupational therapists, registered nurses and care aides can work with you to help you regain your confidence and strength so you can remain living at home safely and effectively.
We will support you to work towards your own goals and help you reconnect to your community.
How to get this service
You will need a referral from the Regional Assessment Service (RAS) to participate in our Home Independence Program.
Please call our Contact Centre on (08) 9242 0242 or 1300 650 803 for country callers (for the cost of a local call) to discuss your needs and help arrange the referral. We’re available to take your call 24 hours a day, seven days a week.
If you participate in this program you will be required to pay a contribution towards your care.
