Available to people living in the Great Southern region of Western Australia, a Transition Care Package can help you regain your independence after a stay in hospital.
A Transition Care Package is a short-term, intensive program that is designed to help you regain your independence and abilities, both in and around the home, when you are released from hospital.
This is a good option if you need more time and support to help you get back to your regular activities.
We have a wide range of services available to assist you during your recovery. This includes:
- a dedicated care manager, who will help you settle back at home, develop a care plan and co-ordinate all aspects of your package, including establishing community support and services and helping you explore your care options after the package ends
- low intensity therapy such as physiotherapy and occupational therapy
- nursing support
- personal care
- domestic help.
We can also provide advice on equipment and assistive technology that can help you with your daily tasks.
How do I get this service?
Transition care packages are only offered to people living in the Great Southern country region of WA. You will need to be assessed by an Aged Care Assessment Team (ACAT) to receive these services.
Your hospital-based health professional or your GP can refer you for an assessment, or you can request an assessment via the My Aged Care portal.
How much does it cost?
You may be asked to contribute to the cost of your care through a daily fee. These will be explained to you for your agreement before any services are delivered.
