Short-term restorative care packages are available to people living in the Great Southern and South West regions of Western Australia.
Restorative care is a great option for people recovering from an injury or illness, or who need support returning home safely from hospital. It is sometimes called reablement.
Short-term restorative care can help you manage new or changing health conditions. The aim is to help you regain or improve your independence, so you can continue to live confidently and independently.
Our packages are:
- designed to help you achieve your set goals
- delivered by a team of people working collaboratively
- provided in your home or in the community
- short-term (up to eight weeks).
How it works
When you start a restorative care package, you’ll work with members of our team to assess your needs and identify your goals.
This will be developed into a detailed care plan. You, your family or your carer will be involved in this process to make sure it’s right for you.
Your plan will then be delivered by a multi-disciplinary team, which can include:
- 'allied health' professionals – such as occupational therapists, physiotherapists, podiatrists, therapy assistants and care aides
- your general practitioner (GP) or medical specialist
- registered nurses
- specialist service providers such as dieticians, optometrists, speech therapists or social workers
- personal care or home maintenance.
We can also provide advice on equipment and assistive technology that can help you with your daily tasks.
Together, we can help you reach your optimum level of independence and wellbeing, so you can continue to live safely at home.
How do I get this service?
These packages are only offered to people living in the Great Southern and South West country region of WA.
You need to be assessed by an Aged Care Assessment Team (ACAT) before you can enter this service. If you think this program may help you, talk to your GP or hospital-based health professional who can refer you for an assessment.
You can request an assessment via the My Aged Care website.
Short-term restorative care is not available if you are already receiving a home care package.
How much does it cost?
You may be required to pay a contribution, which depends on whether you receive care in the community, or in an aged care home. It is calculated as a percentage of the single pension daily rate.
