Helping clients in Victoria with chronic and complex conditions to manage them more effectively from the comfort of their own home.
If you’ve recently been in hospital, this program can help you connect with the support and care you need in the community. The aim is to help ensure you don’t need to return to hospital unnecessarily.
Through Western HealthLinks, you will have:
- 24/7 nurse phone contact through a national call centre
- 7 days per week access to a dedicated Health Navigator
- a personalised care plan
- a networked approach to help you manage your health from home.
Your Health Navigator will co-ordinate your care, both in and out of hospital. They will tailor a care plan based on your needs and goals.
Your care plan will also help identify which services will support you in managing your health. Our Health Navigators work with you and your regular general practitioner (GP) to create and manage this plan.
Through an online patient portal, you and your family can view your plan, as well as scheduled visits from their Health Navigator and any appointments you may have with health care professionals and service providers.
Who can access the program?
Western HealthLinks is designed to help people who:
- are living at home with multiple chronic conditions
- have had multiple visits to hospital
- live in the Western Health Metropolitan Health Service area.
Enrolment for the service is via an eligibility process developed by the Victorian Department of Health and Human Services, and patients will be identified within the hospital system
Priority Response Assessment
Our Western HealthLinks clients are also able to access the Priority Response Assessment (PRA) service between 7am–11pm, seven days a week.
PRA is designed to help you avoid a hospital or emergency department visit by coming to your home within four hours of your request.
Our PRA nurses will do an assessment of your health. They can provide immediate care in your own home to address many conditions. If you need emergency help, they will support you through the process.
Western HealthLinks patients will be identified within the hospital system and you will need to provide your consent to participate in the program.
Western HealthLinks is designed to assist GPs and Health Providers to support their patients living with chronic and complex conditions in the community. Our Health Navigators will work with you to improve communication and integration of services for the client. This will not replace your vital role as the client’s regular GP or Health Provider.
