How to get a Home Care Package
Find out how to access a Home Care Package to help you live a healthy and safe life at home for as long as you choose.
If you’re a senior who wants to continue living at home, but you need regular or co-ordinated support to do so, then a Home Care Package may be the perfect solution for you.
1. Find out if you’re eligible
Before you can receive a Home Care Package, you will have an assessment through My Aged Care, the national aged care navigation portal. They talk to you over the phone and will refer you for a free assessment. There is no charge for this assessment and we can arrange a referral through My Aged Care.
To arrange an assessment, you can:
- give us a call and our friendly team can help get you started
- register with My Aged Care
- call My Aged Care on 1800 200 422 with your Medicare number ready.
2. Have an assessment
An independent assessor will come to your home to get an understanding of your needs. They’ll work with you to identify your strengths, areas of difficulty and your goals.
With a Home Care Package:
- you will be allocated one of four package types – level 1, 2, 3 or 4 – depending on the complexity of your support needs
- you have control over your funds and can decide how to spend it
- once you have a package, it stays in place for the long term.
3. Set up your Home Care Package
After your assessment, you will receive a letter to let you know if you have been approved to receive a Home Care Package. The letter will also confirm the level of package you are eligible to receive.
Once you have your referral code, call us directly. One of our care team members will visit you at home and we’ll work with you to design a care plan that’s tailored to your needs, budget and personal circumstances.
We’re available 24 hours a day, seven days a week.
While you are waiting for your referral code we can support you to access subsidised services through the Commonwealth Home support programme or privately funded services.
Getting to know you is important to us. We will listen to your goals and preferences and as your needs change we can change with you.
There is no cost to have an assessment for a Home Care Package, but if you are eligible, you may be asked to contribute to the cost of your care, where your personal circumstances allow.
