1. Help us understand your needs
There are many options available to support you at home and help you stay independent, healthy and safe. Home Care Packages are just one. We will help you make sense of the options available free of charge. Once we understand your needs we can advise you on the next steps to getting the care you need.
2. Find out if you’re eligible
Before you can receive a Home Care Package, you’ll need to be assessed by a member of the Aged Care Assessment Team (ACAT), who are independent of RDNS. There is no charge for an ACAT assessment. We can help you get a referral to the ACAT by contacting My Aged Care or through your GP.
3. At the assessment
The ACAT will visit you to carry out your assessment. The assessment will take between 45 minutes and 2 hours depending on your needs.
You may have to wait for your assessment. In the meantime, we can offer you private services right away while you wait.
4. After the assessment
The ACAT will write to you to let you know if you have been approved to receive a Home Care Package. If you have been approved, this letter will include the level of package you are eligible to receive.
There may be a wait time between your Home Care Package approval and being assigned a Home Care Package. This wait time will be based on your needs and circumstances. If you’re happy to wait to be assigned a Home Care Package, you’ll receive a letter including your referral code. Contact us directly with your referral code and we will be able to arrange your care.

5. Set up your Home Care Package
One of our care team members will visit you at home to understand your needs and work with you to coordinate your support. Together, we will design a care plan guided by you, and from here your services will commence.
6. Ongoing care and support
Building a relationship and getting to know you is important to us. It means that as your needs change, we can change with you. If you need additional support, we can review your care plan at any time.
