Your care coordinator will also help you get the best value from your package and help you manage your budget effectively. Not only will you be able to contact your care coordinator for advice or if you have a query, you’ll also receive a monthly review meeting to ensure everything is on track.

We’ll look for hazards to ensure you are not putting yourself or others at risk of injury to reduce the risk of a possible fall in the future. We’ll also work with you to help build your strength and confidence to make sure you’re maintaining your independence.

5.   Around the clock support with our local 24/7 contact centre

Our local Contact Centre are available for you 24 hours a day if you have any questions or need urgent support. We’re here for you all day, every day.

6.   Help getting settled back home after a hospital stay

If you go into hospital, we can help you regain your independence upon your return. We have a wide range of services available to assist you during your recovery, so that you can get back to living life to the fullest.
