Home Care Packages are subsidised by the Australian Government and are available in four levels depending on your needs.
- Level 1 - basic care needs
- Level 2 - low-level care needs
- Level 3 - intermediate care needs
- Level 4 - high-level care needs
A Home Care Package might be suitable for you if you need personalised and coordinated care which is more complex than the care available through the Commonwealth Home Support Programme.
To find out if you are eligible for a Home Care Package, you will first need to have an assessment. Assessments are organised by My Aged Care and are free of charge. An independent organisation called the Aged Care Assessment Team (ACAT) will carry out the assessment. To get an assessment, you can either be referred by your GP or you can contact My Aged Care directly on 1800 200 422. We can help you with this referral – contact us.
A member of the ACAT will come to your home to carry out your assessment. This will usually be a nurse, social worker or other healthcare professional. Depending on your location, you may need to wait for your assessment. My Aged Care will inform you of the wait time. In the meantime, if you need services right away, we can help.
During your assessment, the assessor will talk to you about how you’re managing in your day-to-day life. They will get an understanding of what you need help with and what your goals are to enable you to live well at home. The assessment is likely to take between 45 minutes and 2 hours.
After your assessment
The ACAT will write to you to let you know if you have been approved to receive a Home Care Package. If you have been approved, this letter will include the level of package you are eligible to receive.
There may be a wait time between your Home Care Package approval and being assigned a Home Care Package. This wait time will be based on your needs and circumstances.
