HIV care and co-ordination
We provide a specialist human immunodeficiency virus (HIV) service, working closely with other support agencies to support your health and independence.
We work to build comprehensive and co-ordinated care packages for people who are either:
- newly diagnosed with HIV
- living with complex health needs
- new to treatment and care in South Australia.
Our HIV Enhanced Primary Care Coordination Program will support you to liaise with service providers throughout your journey. Our specialist nurses will work closely with you to make sure your care is integrated.
If you require longer term support or education on HIV medication management, you will also be included in this program.
The principles of our services are underpinned by confidentiality, acceptance, care, trust, empathy, respect, empowerment and partnership. We apply these principles to:
- enable positive interactions with people living with HIV
- work towards breaking down the effects of stigma, fear, discrimination, homophobia and prejudice.
Our aim is help you be more independent and able to self-manage your health, in your own home.
