Support for Younger Adults in South Australia
Community Connections Program
The South Australian Government funded Community Connections Program supports people to increase their independence in the home and to build stronger social and community connections. RDNS Silverchain is the Care Partner for metropolitan Adelaide.
How Community Connections works
Community Connections Partners across South Australia provide short-term support for up to 12 weeks or connect you with longer-term support if needed. This may include helping you to take part in local community activities, find new support groups or receive some extra help at home.
Who is Community Connections for?
Community Connections is open to people living in South Australia, who:
- are aged 18 to 64 (18 to 49 for Aboriginal and Torres Strait Islander peoples)
- need short-term support to improve independence or to develop or maintain connections with support networks
- are not eligible for federal programs such as National Disability Insurance Scheme (NDIS) or My Aged Care.
Priority will be given to:
- unpaid carers
- Aboriginal and Torres Strait Islander peoples
- people from new and emerging culturally and linguistically diverse communities
- people who are financially disadvantaged
- people living in communities of persistent or location-based disadvantage
How to make a referral
Whether you are looking for support for yourself, a friend or a family member, or you are a service agency wanting to refer somebody to the program, the Regional Coordinating Partner in your area is your first point of contact. They will facilitate a referral to RDNS Silverchain if you require Care Partner services.
RDNS Silverchain Care Partner services
RDNS Silverchain is the Care Partner for metropolitan Adelaide.
Care Partners provide home-based health support to help participants become more independent. Our experienced allied health team will work with each participant to achieve specific goals. This often includes improving confidence and independence to move around, become more independent in completing daily living tasks and keeping your home safe.
RDNS Silverchain can also facilitate referral to the Department of Human Services, Equipment Program should you require equipment or home modifications.
Referral to RDNS Silverchain Care Partner
You can also refer directly to RDNS Silverchain as all partners can facilitate access to the Community Connections Program. This may be helpful if there is a need for home-based health supports to remain safe at home.
