Priority response assessment in Western Australia

Advanced clinical assessments in the comfort of home

Our Priority Response Assessment is a unique assessment service that provides advanced clinical assessments in your home. This service is designed to help you manage chronic diseases by avoiding unplanned hospital or emergency department visits. 


Our dedicated Priority Response Assessment nurses will assess your clinical status and risks to help determine the best level and setting for the care you need. 

 

Through this service home nursing visits can be provided between 7am and 9pm, seven days a week 


Phone-based nursing support is also available 24 hours a day, seven days a week. 

Who is eligible?

This service is available for people who live in the Perth metropolitan area and have been referred by a medical practitioner.  

How much does it cost?

This is a free service, funded by the Western Australian government.  

Making a referral

Your GP or specialist can refer you to this service. All referrals are assessed for eligibility.
