We believe that everyone deserves to live a healthy, safe and independent life in the comfort of their home, and to stay active and connected within their community.
We provide tailored, personalised care to help you stay at home. We aim to integrate with your existing lifestyle, so that you can feel confident and supported in undertaking normal, everyday tasks. Our approach to home care includes a focus on individual strengths and abilities, offering our support where needed.
If you need a bit of assistance around your home with day-to-day tasks, we can support you. If you have more complex health needs or develop an illness or injury, we'll include our specialist in-home nursing, clinical or therapy services in your care plan.
Government subsidies are available for home care services depending on your personal circumstances. To receive subsidised care, you will first need to have an assessment.
The assessment will be carried out by an independent organisation and will help to identify your needs and the type of care that is right for you.
Waiting times for an assessment vary depending on the type of assessment you need. You may be seen within a few days or it could be a number of weeks or months. In the meantime, we can offer you services right away while you're waiting. Talk to us about our private services by calling our care team on (08) 9242 0119.
During your assessment, the assessor will also talk to you about what makes life enjoyable for you, your individual needs and the goals you have to life your most meaningful life. You'll be asked who you'd like to provide your care - be sure to ask for Silver Chain.
Following your assessment, we'll receive your referral and will be in touch to arrange the care you need.
Our knowledgeable and friendly team can help you arrange an assessment. Contact us today on (08) 9242 0119.
How much do home care services cost?
The Australian government pays for the bulk of home care services in Australia, however, you may be asked to contribute towards the cost of your care if you can afford to do so. You will never be denied the care because you cannot afford it. The funding program or package through which you access aged care services at home will determine how much you will need to pay.
If you are not eligible for government funding, need services while you wait for a government assessment, need short-term home care or would like to 'top up' your funded package of services, we provide all our home care services on a private, fee-paying basis. See private home care services for more information.
What is a Home Care Package?
A Home Care Package is a 'package' of personalised services to support you to stay healthy and safe in your own home as you get older. Home Care Packages are funded by the Australian government and are available in four levels depending on your needs. A budget will be allocated for you to use on support and services that suit your needs, provided by Silver Chain.
Home Care Packages are tailored to you, so you can choose the services that best suit your needs and preferences. For example, a package could include just a few hours of care each week, or it could include more regular support. Find out more about Home Care Packages.
Who is eligible for government funding?
Before you can receive government subsidised home care, you will need to be assessed by an independent organisation. There is no charge for an assessment. You can get a referral for an assessment either through your GP, by contacting My Aged Care or call our care team on (08) 9242 0242 and we will be happy to help you organise one.
