Chronic and complex disease management in New South Wales

We provide a free care coordination service in the following regions – Wollongong; Shellharbour; Kiama; Shoalhaven; Jervis Bay; Eurobodalla; Bega Valley; Snowy Monaro; Queanbeyan Palerang; Goulburn Mulwaree; Yass Valley and Upper Lachlan Shire.

People outside of these regions should speak to their GP or care provider about services available to them.

Support for people with chronic conditions

We know that ongoing management is important for people with chronic health conditions and complex care needs. We know that sometimes getting the right support can be hard – and you might not know who to turn to for help. This is where we can support you to ensure you get the services you need.

Your GP might be talking to you about things like getting your blood pressure down. You might also have lifestyle goals – maybe you’d like to get out more to do the things you enjoy, play with your kids or grandkids, or be fit enough to walk your daughter down the aisle. We can take the time to really understand what you need and together we’ll set up a plan and help to get you there.

Speak with your GP about referring to our service. We will assist you in the coordination of your care, including assisting with your health care appointments and working with you on a plan to maintain your best health.

Support for adults with chronic conditions

We understand that every person has different needs and different wishes and that's how our team of caring experts can help.

If you experience limitations as a result of a chronic condition or have a life limiting illness and wish to receive care at home, support is available.

We work together with a range of partners to co-design and deliver innovative care programs. Our locally based health and community partners include, for example WA Country Health Service, WA Primary Health Alliance and Asthma WA.

Our capability and expertise enables us to provide a diverse offering of programs across different chronic conditions.

Health Navigator Program

Our Health Navigator Coordinators work with people who have chronic health conditions to help you identify health concerns and problems, and develop a personalised plan to improve your health and wellbeing. This might include linking you with other services, providing you with information about your conditions or supporting you to make a change in your life. 

We work alongside health care professionals you are already seeing, such as your GP. You can choose to access this service over the phone or by using video conferencing.

Chronic Obstructive Pulmonary Disease 

For people with chronic obstructive pulmonary disease (COPD), this program helps you to get back on your feet after a stay in hospital by working towards your goals, as well as providing support and education.

Our care team will complete a holistic assessment with you to ensure you have appropriate referrals, prescriptions and the supports you need for you respiratory condition and any other health conditions you may have.

Following this, Asthma WA’s Respiratory Health Team will work with you to provide COPD education and support for a period of three to six months.

This program is supported by WA Primary Health Alliance.
