Here you can access forms to refer patients with a life-limiting illness or condition to one of our palliative care services. We work across eight Local Health Districts in New South Wales.

These referral forms are available to Local Health Districts (LHDs) and general practitioners.

Palliative Care Home Support Packages (PEACH)

The PEACH program provides personalised, short term packages of care to support individuals at the end of their life. This is available to residents in parts of Sydney and surrounding areas.

Last Days of Life

This home support service is available to people of all ages, who are in the final stage of a life-limiting illness who wish to die at home, or stay at home for as long as possible. Services are provided in conjunction with, and in the areas of the Northern NSW LHD, NSW Mid North Coast LHD and Hunter New England LHD

Community Palliative Care Service

  • Patients are eligible for this service if they:
  • live within Western Sydney LHD
  • have an advanced, progressive, life-limiting condition
  • are over 18 years of age, or a patient moving from child to adult health services
  • have been consulted about being referred to the service.
