Last Days of Life (regional)
We support palliative care packages provided across regional New South Wales through the Last Days of Life Program.
This home support service is available to people of all ages, who are in the final stage of a life-limiting illness who wish to die at home, or stay at home for as long as possible.
Our home support nurses specialise in end-of-life care and are available 24 hours a day, seven days a week. They work with your general practitioner (GP) and community nurses to provide qualified and reliable support in the home, including:
- a daytime visit by an assistant-in-nursing to provide daily personal care and/or respite for carers
- an evening visit by a registered nurse for clinical care and support
- overnight support a registered by nurse who can provide clinical advice and support via the phone or video link (on a video device provided by Silver Chain).
Services are provided in conjunction with, and in the areas of these local health districts:
- Northern NSW LHD
- NSW Mid North Coast LHD
- Hunter New England LHD
