Community packages (ComPacks)
Our ComPacks program provides short term support for people being discharged from NSW public hospitals with daily tasks.
We facilitate the safe and early discharge of eligible clients from hospital by providing access to a short term package of care. We will work with you to identify what support you need to regain your independence and prevent re-admission to hospital.
How does it work?
One of our ComPacks Case Managers will work with you over a six week period to develop strategies to address short term functional challenges, as well as facilitate access to longer term support where required. Support provided may include:
- personal care
- domestic assistance
- social support
- in-home respite.
How do I get these services?
You will be referred to our ComPacks program by your hospital. Eligibility is based on your need for immediate access to case management and community support that can't be access through other channels.
Making a referral
If you have a patient you would like to refer to Silver Chain, complete the ComPacks referral form.
