A new virtual care home monitoring program for COVID-positive South Australians is providing clinical assessments and support for people who have medium to high risk symptoms from COVID-19.
RDNS Silverchain is proud to support the South Australian Government in delivering the COVID Home Telephone Monitoring Program.
After an initial clinical and wellbeing assessment, COVID-positive patients can be referred to the program to receive daily monitoring on their health and wellbeing status and ensure any deterioration in condition is picked up and managed.
Dale, an RDNS Silverchain Health Navigator, said the team makes regular contact with patients, assesses their symptoms and how they are managing at home, and escalates their needs as required.
“Patients have told me they feel reassured when they speak to our nurses, and they know they can also contact us directly if they have a question outside of the regular calls,” Dale said.
“As well as the physical health checks we do on each call, many patients tell me they appreciate having someone to talk to, particularly if they live alone.”
For more than 125 years, RDNS Silverchain has provided high-quality, safe health and aged care services to multiple generations of South Australians, supported by more than 1,000 employees across the state. RDNS Silverchain has been part of not-for-profit Silverchain Group since 2011.
