For more than 110 years, Silver Chain has provided safe and trusted hospital-level care in the home and community.
In Perth, this is delivered through our unique Home Hospital model of care. With Home Hospital, you receive the same level of care as you would in a hospital, at home.
Our team of skilled doctors, nurse practitioners and qualified nurses visit you at home to provide care for a range of acute conditions.
Care in the community
Home Hospital nurse Katrina says working in the community has great benefits – for staff and clients alike.
“One of the best things about working for Silver Chain is being able to focus on each client individually,” she said.
“With Home Hospital, you have the chance to give them your undivided attention.
“It’s a great feeling to see them heal and get back to feeling well,” Katrina added. “There’s a real family feel across the service.”
From hospital to home
Most of us feel more comfortable at home and for our client Elaine, she just wanted to get things back to normal after six long weeks in hospital.
“I just wanted to be back in my own bed,” she said. “At first, I was quite scared being discharged from hospital because I wasn’t sure how to take care of my dressings.
“But Silver Chain called the next morning and were such a great help – it eased my mind straight away.
“The nurses go out of their way to help you and I know if I need any help, I just have to ring them. I cannot speak highly enough of them!”
Hospital in the home
In Queensland, we provide ‘Hospital in the Home’ on the Sunshine Coast in partnership with the Sunshine Coast Hospital and Health Service.
And at RDNS in South Australia, our nurses can remotely remind clients to take medication or observe people taking tablets or insulin as part of a virtual health service using wireless video technology. How does it work?
Your GP or hospital will refer you for hospital-level care that can be safely provided in your own home.
