Silver Chain now has a Health Navigator Nurse based at the Albany Health Campus, to support those living with chronic conditions of diabetes, heart failure or heart disease or long term lung conditions to manage their health.
Kerri Lockhart commenced in the Health Navigator Nurse role in March this year and is already supporting clients referred to the service to access the care they need in the community.
Silver Chain Director of Clinical Services, Country Services, Lesley Pearson, said Kerri works in partnership with the coordinators from the Virtual Health Navigator program already underway in the Great Southern and Wheatbelt regions of Western Australia.
“Kerri will follow up on referrals initiated whilst the client was in hospital to ensure they can attend. Where appropriate she will commence a self-management program to support them in managing their condition and address any barriers,” Lesley said.
“After discharge from Albany Health Campus Kerri then supports the client to transition to the Virtual Health Navigator program.”
Silver Chain is working in partnership with the Southern Inland Health Initiative to trial the Health Navigator Nurse role. Client who have presented to Albany Health Campus and live in the great southern region are eligible for the service.
