Often when older people feel they need a little extra support they’re not sure where to start because navigating the aged care system can be challenging and confusing.
For those who want to continue living at home, a Home Care Package could be a great place to start.
Home Care Packages are a government funded ‘package’ of care services that allow Silver Chain clients to enjoy a sense of wellbeing and independence in their own home, for as long as they choose.
From assistance with showering or grocery shopping, to help getting around their local community, clients are supported to live at home and keep doing what they’ve always done.
When a client receives a Home Care Package, they are the decision maker. They have more choice, more control and access to a full range of aged care services.
A client also chooses how their funds are used and how involved they want to be in coordinating their care. We work in partnership with our clients to give them support and advice – as much or as little as they want or need – as part of our Case Management service.
Through the ongoing delivery of tailored services to meet individual needs, our clients are empowered and supported to live their best lives in the community.
Lotti, 96 years of age, a Silver Chain Home Care Package client, shared why living in her home of 20 years is so important to her wellbeing.
“I love my home. There are lots of birds that love my little garden. A few months ago, I found a nest in my pergola and there were three chicks. I love sitting in my back courtyard too, listening to the children play in the school next door. I wouldn’t be able to do this if I were in a nursing home."
Before discovering Home Care Packages, Lotti’s daughter-in-law, Jessica, had no idea that support was available for Lotti at home.
“Lotti is quite independent and only needs a little bit of help, but because we work during the week, it’s hard for us to provide that additional support she needs,” Jessica said.
“We thought we’d have to put Lotti in a nursing home. We’re thrilled.”
