Transitioning to Consumer Directed Care
Home care in Australia is changing. Clients now have more choice, more control and easier access to a full range of aged care services.
These changes are the result of Federal Government reforms and they start with the introduction of Consumer Directed Care (CDC) into Home Care Packages.
This means you’re in charge. You can choose to have more control and say in the type of care you need to help you live independently.
At Silver Chain we fully support these changes. We understand the importance of living your best life at home and we’ve been supporting people to do just that for more than 110 years.
An important element of CDC is that it’s all about building on what you can do rather than focusing on what you can’t do, and here at Silver Chain, this thinking forms an integral part of our home care offering.
Silver Chain General Manager, Home Support Service, Anne-Marie Cox, said CDC and home care in general is now moving towards consumer participation – that is, empowering you to have more say in the care and services you access, how they are delivered and by whom, so that you can continue to live independently at home.
“At Silver Chain, no matter the support you think you need, we work with you to understand your goals in life. We then create a care plan that focuses on what you can do and build on your strengths, helping you regain confidence in the activities of daily living,” Anne-Marie said.
“Our approach to care empowers you and enhances your independence by building on your capabilities and strengths in all aspects of your life. That way we can help you to restore your physical, functional and social health.
“We look at the whole you, and provide a range of solutions and services to suit your needs. That can vary from help with tasks around the home or creating a strength based exercise program for you to nursing care, linking you back into your community or connecting you to one of our Social Centres. The solutions available to help you live your best life at home are as individual as you are!
“You may not always need us, but when you do we will be there for you, supporting you on life’s journey. So next time you need help to remain living independently at home, think of Silver Chain, as we can be there right by your side,” Anne-Marie said.

The difference a Home Care Package can make…
Mrs O’Connor understands the benefits of Consumer Directed Care.
She receives nursing visits daily and after being discharged from hospital, her family arranged personal and domestic care for her too.
This was expected to be a short term solution while her family was looking for a residential care placement. They thought their beloved mother couldn’t manage at home with her dementia and declining mobility.
However, with the care she is receiving at home, Mrs O’Connor is gradually regaining strength and has managed to catch the bus into the city a number of times on her own, an activity she carried out regularly in years gone by.
Her family are no longer looking for a residential care placement and have instead arranged for her to continue to receive care under a Home Care Package, which now incorporates a CDC approach and will support her to remain living at home.
