Silver Chain is evolving its Respiratory Service from one that provides clients with an oxygen unit and some clinical support to one that helps clients manage their chronic condition by focusing on education, self-management and increased clinical support.
Our vision for this service is to provide greater support to clients by assessing and monitoring their condition more regularly, and by recognising any signs and symptoms of deterioration so we can respond quickly and ultimately improve their quality of life.
To help meet the client’s need, we will also connect them with the many services we offer including Home Hospital, Hospice Care Service and Home Support Services.
Our goal for the service is a reduction in the number of presentations and admissions to hospital.
Silver Chain Clinical Nurse Consultant Manager, Kirsty Tilden, will be overseeing a six month trial of the new look service with a group of Respiratory Service clients.
“Trial participants will receive more regular visits from one of our specially trained respiratory nurses, who will review their condition and progress and provide more feedback to their GP and specialist,” Kirsty said.
“The nurses will also work to improve links and referrals to both Silver Chain and external services to better support the client in the community.
“Some of the clients participating in the trial will also receive telehealth technology to assist them in monitoring and managing their symptoms.”
All Respiratory Service clients will be asked to take part in short monthly phone calls and three monthly written questionnaires, to assist us in measuring the success of the trail.
“We are really excited about what this trial could mean for our clients in the future. Ultimately, we want to be able to provide better support to people in the community living with respiratory conditions, through our Respiratory Service,” Kirsty said.
