For City of Albany Mayor, Dennis Wellington, recovery from a life-threatening health condition was made a lot easier with the help of Health Navigator.
When Mr Wellington was discharged from hospital after three days in ICU, like many others before him he lacked support and direction to get his health back on track.
“When I was discharged from hospital, all I needed was a liaison person, someone to check in on me and find out how I was progressing,” Mr Wellington said.
“Instead I was left feeling quite forgotten about, and I still had concerns about my health.”
Mr Wellington was told about Silver Chain’s Health Navigator service and a referral was arranged for him.
The Health Navigator service uses phone and telehealth technology to support people with chronic conditions to effectively self-manage their health.
“One of the Health Navigator coordinators asked questions about my condition and provided advice about the services available to me,” Mr Wellington said.
“She then arranged all the necessary referrals and appointments. She was really helpful and took the confusion and stress out of my follow-up care.”
People in the Wheatbelt and Great Southern living with chronic conditions of diabetes, heart failure or heart disease or long term lung conditions and those at high risk of developing these can access the Health Navigator service to manage their health.
