Every year, we strive to deliver a high standard of care so our clients can stay at home and live full and independent lives.
During the past financial year, our 3,200 Silver Chain Group staff and 380 volunteers provided 1,732,862 hours of care and 2,597,388 occasions of service to 82,504 clients across Australia.
These figures appear in our digital annual report, Silver Chain Group - A Year in Review 2014/15, along with an overview of our national achievements, service highlights and a financial summary.
Some of the highlights that appear in the report include:
Silver Chain celebrated 110 years of caring for Western Australians. This milestone was marked with a new brand campaign – A shared journey – that focuses on our clients and demonstrates that no matter what stage they’re at in life, we will be there by their side.
In New South Wales, we provided palliative care services to almost 900 clients. Our work enabled 684 of them to die in their place of choice, at home, with the remainder able to remain at home as long as possible.
We continued to establish our Hospital in the Home (HITH) service in Queensland and in March 2015 we expanded our reach and impact with the commencement of our HITH service in the Mackay Hospital and Health Service.
In South Australia, we have actively encouraged our clients to assist us to design our services – seeking their feedback on new service models and we’ve addressed a number of processes to improve the client experience.
The Home Hospital service in metropolitan Perth, Western Australia, has grown considerably with Silver Chain becoming the provider of Home Hospital services for Fiona Stanley and Royal Perth Hospitals.
We began providing services in the north west of Western Australia, the Pilbara region, with services transitioned from both WA Country Health Service and Southern Cross Care to Silver Chain.
We prepared our organisation for the introduction of the Federal Government’s Aged Care Reforms, in particular Consumer Directed Care into Home Care Packages and the introduction of the Commonwealth Home Support Program.
