Home Care Packages

We are challenging and advocating for an integrated care approach for all Australians in the health, community and aged care sectors. Whether at home or facilities based, we work to provide access to quality in-home care.

While we don't currently provide Home Care Package services in your area, we are committed to ensuring that people have access to the right information regarding their Home Care Package options where and when they need it. 

We have compiled a list of considerations that we believe are important for you when seeking care. We hope this helps you on your care journey and as we grow and expand, so will our ability to provide more choice for quality home-based care in your area.

What is a Home Care Package?

Home Care Packages help you receive the care you need to live independently. These are tailored packages of services designed to give you choice of services you need to continue living your life, in your own home, confidently and comfortably for as long as you can.

What can be included in a Home Care Package?

Personal care and wellbeing

In-home personal care and wellbeing services can help provide you with confidence that comes from both you and your home looking and feeling their best. Tailored around support that best suits your needs, this can include services from personal care to assistance with shopping and meal preparation.

Health care

In-home health care services can be provided by trained nurses and allied health professionals to help you get back on your feet and keep you at your best, living safely and confidently at home.

Home and garden support

Help around the house and garden can be vital to maintaining your independence. Services may include help with housework, meal preparation, assistance with shopping, gardening and other everyday activities.

Transport

Through safe and reliable transport, getting out and doing the things you love helps keep you happy and connected.

Community and companionship

Whether it’s enjoying hobbies, getting out into your community or just having a good chat, companionship and support can be provided at home or in your community.

Your support network

Caring for someone can be a full-time commitment - carers need support as well. Respite services offer carers a chance to take some time for themselves and recharge so that they can continue to offer their best care.

Who's eligible?

Generally to be eligible for services through a Home Care Package, you will need to be:

• 65 years of age or over, or 50 years of age and over for Aboriginal and Torres Strait Islander people
• someone who needs coordinated care services to help them stay at home.

The level of your Home Care Package is based on your individual care needs, determined by a face-to-face assessment with a professional assessor.

How to apply

Speak to your health care professional

Speak to your GP or health care professional who can help answer your questions about Home Care Packages, including what services are available and how to get assessed. Alternatively, you can contact My Aged Care directly to arrange an Aged Care Assessment Team (ACAT) assessment. Have an ACAT assessment

A professional assessor will work with you to assess your care needs. They'll help identify your strengths, areas of difficulty and help you set goals.
