Depending on your unique needs and preferences, we'll help you work out what's available, how to get started, how much it costs and answer any other questions you may have, so it's easy and hassle-free for you to make informed decisions and receive the care you need.

 

 

Depending on your personal circumstances, you may be eligible for subsidised home care. 

There are two types of home care that are subsidised by the Australian Government. These are Home and Community Care (HACC) and Home Care Packages. Both programs are designed to enable you to live independently at home and meet your changing needs as you get older.

People of all ages can receive our services. We provide in-home aged care, but we can also care for children and adults with illness or injury, and provide support and advice for family, friends and carers through a range of situations.

There are many cases where the full cost of the service will be covered by us, at no charge to you. For other situations you may be required to pay a contribution to your care or you may choose to use our private services on a fee-paying basis. To receive Government funding for home care, you will first need to be assessed.

The assessment will be carried out by an independent organisation. For Home and Community Care services, you will be assessed by the Regional Assessment Service (RAS). For Home Care Packages you will be assessed by the Aged Care Assessment Team (ACAT).

Waiting times for assessments vary depending on the type of assessment you need. If you have to wait for an assessment and need support right away, we can help you. We can also organise an assessment for you.

To receive our health care services, you may need a referral from your doctor or other health care provider.

To receive our health and nursing care services, you will need to be referred from your GP or health practitioner (which includes specialist, medical practitioner, St John Ambulance or nurse in community setting, Health Direct, Residential Aged Care Facility or hospital).

Silver Chain will then partner with your GP or health practitioner to coordinate the next stage of your care.
