Palliative care

We believe that every Australian deserves the right to decide how and where they live their last days. Our palliative care services provide comprehensive care to people in the community, allowing clients with life limiting illness to receive care in their own home. Care is person centred, based on the needs and goals of the client, their caregivers and family.

Our team of registered nurses and doctors currently provide in home palliative care services in Western Australia, South Australia and New South Wales.

Wound care

Our specialist wound care nurses use the latest research, technology and treatment methods to help clients manage wounds at home.

We treat wounds including abscesses, skin tears, pressure injuries, burns and ulcers, as well as complex and chronic wounds using advanced technology such as low frequency ultrasonic debridement.

We are leaders in wound care in Australia:

  • contributing to wound care research
  • advancing knowledge in wound assessment, management and healing through our Wound Management Master Class for clinical professionals
  • our wound expert Keryln Carville wrote the Wound Care Manual used by both medical professionals and students in Australia, and presents her findings around the world.

Our research includes working with the Wound Cooperative Research Centre to research and investigate wound solutions.

Home Hospital

We are Australia’s leading home hospital, providing hospital level care to clients in their home. Our goal is to provide patients with the option of safe, quality acute care in the comfort of their own home.

Care is provided in the patient’s home by registered nurses, supported by allied health professionals, enrolled nurses, assistants in nursing and personal carers. Our multidisciplinary teams provide care that is comparable to a traditional inpatient hospital setting. These clients would otherwise need to be admitted to hospital to receive the same care.

We have been a pioneer in the development of alternatives to hospital-based care with our innovative ‘virtual hospital’ service model. We have been providing in home hospital level acute care in Perth since 2009, with a service delivery capacity of over 160 beds.

Chronic and complex services

Our chronic and complex services deliver comprehensive, coordinated care in the community, enabling Australians with chronic conditions to spend more time at home while reducing hospital admissions.

We partner with General Practitioners (GPs) to develop comprehensive patient care plans and screen patient risk, including monitoring and intervention as required. Care is then provided in the home or community, with referrals made where appropriate.

Our Health Navigation service is a 24/7 contact point linking GPs, patients and their families, coordinating patient care in and out of hospital.

Aged care in the home

We believe that everyone deserves to live a healthy, safe and independent life in the comfort of their home, and to stay active and connected within their community. We provide tailored home care to help our clients stay there.

Our trusted network of skilled specialist and dedicated home support staff provide the care our clients need to continue to live at home. We provided this care to 25,000 clients in 2017/18 through Home Care Packages, the Commonwealth Home Support Programme, Home and Community Care Program, National Disability Insurance Scheme and private services.

Our home care services include:

  • home and garden support
  • support for family and carers.
