Referral

A prospective client need not have exhausted all treatment options before accepting referral to our services. For many, a palliative care approach may be introduced as part of the ongoing and comprehensive medical care being received, while others may need only occasional specialist services.

The Hospice Care Service only accepts referrals from medical practitioners. This also applies to current Silver Chain clients.

Doctors can make a referral 24 hours a day by calling Silver Chain on (08) 9242 0242 and completing a referral form.
