Silver Chain delivers nursing services in the comfort of the client’s home or at one of our friendly service centres. Our nursing team works closely with the client, their family and other health care providers to deliver a care plan which optimises the client’s overall health and well-being.

Silver Chain’s nursing services are delivered by experienced registered and enrolled nurses in a setting comfortable for the client. Our nursing team has access to clinical specialist support and use the latest nursing techniques.

Types of care available:

• Wound care
• Continence management
• Home Oxygen
• Diabetes management

The cost and scope of the treatment provided will be based on the client’s individual needs and situation. Co-contributions are available to eligible Silver Chain clients.
