The Development and Delivery of a Training Package for Care Co ordinators to Deliver a Modified Home Independence Program (HIP) Service Model

This project will modify the existing Home Independence Program (HIP) service model, and develop and deliver a training package to broaden the profile of the workforce delivering HIP.
The project has three phases:
- Phase 1 - Modify the existing HIP service model to be delivered by Care Co-ordinators
- Phase 2 - Develop the selection criteria for the Care Co-ordinators
- Phase 3 - Develop and deliver the HIP Care Co-ordinator training
Project update
The project is progressing according to plan. Internal and external steering groups have been established and meetings held. The Project Manager has attended three collaborative workshops with HWA and regular teleconferences with other members of the collaborative continue. An interim report was written and submitted to HWA in December 2010.
The modified service model has been developed following an extensive review of the literature and review of the current HIP model including roles and responsibilities within the Independence Team. Work is now commencing on development of the selection criteria, position descriptions and duty statements. A workshop was held in January to commence development of the training package.
