Diabetes Management

We offer a specialist diabetes service aimed at assisting people with diabetes to take an active and informed part in the management of their condition.

The diabetes team consists of clinical practice consultants, clinical nurses and podiatrists, who work in partnership with General Practitioners, endocrinologists and general medical specialists, hospitals and community based services in the provision of evidenced based diabetes care for children and adolescents, their families and adults with type 1 or type 2 diabetes.

We provide services based on the “Diabetes Self Care – 7 Steps to Success” program, for people who are newly diagnosed or who are experiencing challenges with any aspect of their diabetes management.

Specific post acute paediatric services are provided to children, their families, schools and child care services.
