Remote area health
Remote Area HealthRemote Health Services plan and co-ordinate a range of services including emergency services, non emergency services, chronic disease management, disease control, prevention and health promotion services and child health development screening. Remote Health Services maximise the health potential of the communities they serve within the resources available and acknowledge services provided are linked to community priorities. Each Remote Area Nursing Post (RANP) provides a selection of the services listed below. However not all aspects of every service is expected to be provided in each location.

DIAGNOSIS AND TREATMENT
|
Non-urgent outpatient attendance (Monday to Friday).
Home base and weekend services are available based on demand and capacity.

Non-emergency consultative services including:
Assessment Treatment Review
Follow up care post discharge from acute hospitals and emergency services
Referral for the ongoing treatment of a health condition that requires attendance at an out patient clinic.

Emergency attendances will receive:
Assessment/Triage Treatment Referral Transfer if required
Accepted response time – 20 minutes.

PREVENTION & PROMOTION |
|
Maternal and Child Health
|
Antenatal and postnatal women and their children up to the age of 5 years.
|
Antenatal and postnatal education and support.
Screening of children from 0-5 years in accordance with the Department of Health (DOH) WA screening program.
Provision of immunisation and communicable disease prevention and management.

School Health
|
Primary and secondary school clients aged 5-17 years
|
Regular screening in accordance with the DOH screening program.
Ongoing health education sessions at schools.
Special clinics such as enuresis and ear clinics.
Provision of immunisation and communicable disease prevention and management.

Chronic Diseases Management (CDM)
|
Chronic diseases |
In conjunction with a medical practitioner this service provides treatment/monitoring support / advocacy and self care management. CDM Services may target population with Diabetes, Ischaemic Heart Disease and Chronic Obstructive Pulmonary Disease. These services may be provided in the community, over telephone or in clinic setting.
Outreach services to Aboriginal adults and other community in need.

All the population who have a disease or who have a positive diagnosis of the disease |
In conjunction with a medical practitioner this service provides: screening, assessment, referral, treatment; monitoring, health promotion, contact tracing and counselling to effect detection, treatment and control of communicable diseases.

Primary Health |
Adults and children living in the community. |
In collaboration with DOH and WACHS, to provide health promotion and prevention.
|
Note: Community health services, where agreed to be provided, will be in accordance with State and National policies e.g. Community Nursing Policy, Child Screening and Surveillance Program.
